Sinus Augmentation

Steps Involved in IVF:

Procedure Description

Sinus augmentation is an intricate procedure that begins with careful planning. Prior to the surgery, the patient undergoes a series of diagnostic tests, including X-rays and CT scans, to assess the anatomy of the sinuses and upper jaw. This allows the dental surgeon to evaluate the bone structure and determine the best approach for the procedure. The most common reason for sinus augmentation is bone loss in the upper jaw, which can occur due to tooth extractions, gum disease, or natural aging.

The sinus augmentation procedure involves making a small incision in the gum tissue, exposing the bone underneath. The dental surgeon then creates an access point in the bone, gently lifting the sinus membrane upwards. Once the sinus membrane is lifted, bone graft material is inserted into the newly created space. This bone graft can be derived from the patient’s own body (autograft), a donor source (allograft), or synthetic materials designed to mimic bone tissue.

Once the bone graft is in place, the area is stitched closed, and the healing process begins. Over the course of several months, the grafted bone integrates with the existing jawbone, increasing its height and volume. This process is known as osseointegration and is crucial for ensuring that the dental implants have a strong, stable foundation. Depending on the extent of the augmentation and the individual patient’s healing process, dental implants can typically be placed 6 to 12 months after the sinus lift.

Procedure Duration

The duration of a sinus augmentation procedure can vary depending on the complexity of the patient’s case. The actual surgery usually takes between 90 minutes to two hours. However, the overall treatment timeline, from initial consultation to full recovery, can span several months.

  • Initial Consultation and Diagnostic Imaging:

Before the procedure, the dentist or oral surgeon will conduct a thorough examination, including X-rays and CT scans, to evaluate the anatomy of the sinuses and the upper jaw. This step ensures the doctor has a detailed understanding of the bone structure and can plan the sinus lift accurately. The consultation phase typically takes a couple of weeks, depending on the availability of diagnostic tools.

  • Surgery and Immediate Recovery:

The surgical procedure itself is performed under local or general anesthesia and typically lasts between 90 minutes to two hours. After the surgery, patients can expect some swelling and discomfort, which can last for a few days. Pain management and anti-inflammatory medications are commonly prescribed to help ease the recovery process.

  • Healing Phase and Osseointegration:

The healing process is a critical part of the sinus augmentation procedure. It generally takes between four to six months for the bone graft to integrate fully with the existing jawbone. During this time, patients are advised to avoid any activities that may disrupt the healing process, such as blowing their nose, sneezing forcefully, or engaging in strenuous physical activity.

Benefits of Sinus Augmentation

  • Improved bone structure for dental implants.
  • Enhanced stability and longevity of dental implants.
  • Restores functionality in the upper jaw.
  • Prevents further bone loss by stimulating bone growth.
  • Provides a more aesthetically pleasing outcome for dental restorations.

Risks & Considerations

  • Sinus membrane perforation:
One of the most common risks of sinus augmentation is the accidental perforation of the sinus membrane during surgery. If this occurs, the surgery may need to be postponed until the membrane heals.
  • Infection:
As with any surgical procedure, there is a risk of infection. Patients must follow their dentist’s post-operative care instructions carefully to minimize this risk.
  • Excessive bleeding:
While rare, excessive bleeding can occur during or after the procedure. It is crucial for patients to inform their surgeon of any medications or conditions that may affect blood clotting.
  • Bone graft failure:
In some cases, the grafted bone material may not integrate properly with the existing bone, resulting in the failure of the augmentation. Additional surgery may be required to correct this.
  • Chronic sinusitis:
Some patients may experience chronic sinus infections or inflammation following the procedure. If this occurs, medical intervention may be necessary to resolve the issue.

How to Choose the Right Doctor and Hospital

Choosing the right doctor and hospital for a sinus augmentation procedure is crucial to achieving the best possible outcome. First and foremost, it is essential to seek out a dental surgeon who is experienced in performing sinus lift surgeries. Research their credentials, including their education, years of practice, and any additional training in sinus augmentation or implant dentistry.

When selecting a hospital or clinic, consider facilities that are equipped with advanced dental technology and follow international standards of care. A well-equipped clinic with state-of-the-art imaging tools, sterile operating conditions, and experienced staff can make a significant difference in the success of the procedure. Additionally, look for clinics that offer comprehensive aftercare services, as this will ensure proper monitoring of your healing process and minimize the risk of complications.

Frequently Asked Questions

What actually happens during hyperstimulation of the ovaries?

The patient will take injectable FSH (follicle stimulating hormone) for eight to eleven days, depending on how long the follicles take to mature. This hormone is produced naturally in a woman’s body causing one egg to develop per cycle. Taking the injectable FSH causes several follicles to develop at once, at approximately the same rate. The development is monitored with vaginal ultrasounds and following the patient’s levels of estradiol and progesterone. FSH brand names include Repronex, Follistim, Menopur, Gonal-F and Bravelle. The patient injects herself daily.

What happens during egg retrieval?

When the follicles have developed enough to be harvested, the patient attends an appointment  where she is anesthetized and prepared for the procedure. Next, the doctor uses an ultrasound probe to guide a needle through the vaginal wall and into the follicle of the ovary. The thin needle draws the follicle fluid, which is then examined by an embryologist to find the eggs. The whole process takes about 20 minutes.

What happens to the eggs?

In the next step, the harvested eggs are then fertilized. If the sperm from the potential father, or in some cases, anonymous donor, has normal functionality, the eggs and sperm are placed together in a dish with a nutrient fluid, then incubated overnight to fertilize normally. If the sperm functionality is suboptimal, an embryologist uses Intracytoplasmic Sperm Injection to inject a single sperm into a single egg with an extremely precise glass needle.  Once fertilization is complete, the embryos are assessed and prepared to be transferred to the patient’s uterus.

How are the embryos transferred back to the uterus?

The doctor and the patient will discuss the number of embryos to be transferred. The number of successfully fertilized eggs usually determines the number of eggs to be placed in the uterus. Embryos are transferred to the uterus with transabdominal ultrasound guidance. This process does not require anesthesia, but it can cause minor cervical or uterine discomfort. Following transfer, the patient is advised to take at least one days bed rest and two or three additional days of rest, then 10 to 12 days later, two pregnancy tests are scheduled to confirm success. Once two positive tests are completed, an obstetrical ultrasound is ordered to show the sac, fetal pole, yolk sac and fetal heart rate.

Embryoscope©

Built into this technology there is a microscope with a powerful camera that allows the uninterrupted monitoring of the embryo during its first hours of life. In this way, we can keep a close eye on the embryo, from the moment when the oocyte is inseminated and begins to divide into smaller and smaller cells, until it can be transferred to the uterus.
