Immediate Denture, Maxilliary

Steps Involved in IVF:

Procedure Description

An immediate denture, specifically for the maxillary (upper jaw), is a dental prosthesis that is inserted immediately after tooth extraction. This type of denture allows patients to have their new teeth in place right away, avoiding the embarrassment of having missing teeth during the healing process. Immediate dentures are designed to mimic the natural appearance of teeth and are usually custom-made to fit each patient’s mouth.

The process begins with a consultation with a dental specialist, during which impressions of the patient’s mouth are taken before the teeth are extracted. These impressions are then used to create a temporary denture, which will be placed immediately after the teeth are removed. Since the immediate denture is inserted right after the extraction, patients leave the dental office with a full set of teeth.

The design of an immediate maxillary denture takes into account the changes that will occur in the gum tissue as it heals after the extraction. The initial fit may feel snug, but as the gums shrink during the healing process, adjustments may be required to maintain comfort and functionality. After about six months, once the gums have fully healed, a permanent denture or other restorative option may be fitted, ensuring long-term comfort and aesthetic results.

Procedure Duration

The process of getting an immediate maxillary denture can be broken down into several stages, each with its own timeline. The initial consultation and impression phase typically takes one or two visits. During these visits, the dentist will assess the patient’s oral health and take impressions of the upper jaw, which will be used to fabricate the denture. This phase can take anywhere from one to two weeks, depending on the complexity of the case.

The next phase involves the tooth extraction and immediate denture insertion, which usually takes place in a single appointment. The length of this procedure depends on how many teeth are being removed, but on average, it lasts about one to two hours. During this appointment, patients may receive local anesthesia or sedation to ensure comfort during the extractions.

Recovery from the procedure varies from patient to patient. Initially, patients may experience swelling, discomfort, and difficulty eating as their gums heal. However, most patients can return to normal daily activities within a few days. Full recovery, which includes gum healing and adaptation to the denture, can take several months. Adjustments to the immediate denture may be necessary during this time to ensure a proper fit as the gums change shape. After about six months, a more permanent solution can be considered.

Risks & Considerations

  • Gum Shrinkage:
Since gums shrink after tooth extraction, the immediate denture may require multiple adjustments to maintain a proper fit as the healing process progresses.
  • Initial Discomfort:
Patients may experience discomfort, swelling, or soreness in the initial weeks after the procedure as they adjust to the denture.
  • Altered Speech:
It may take some time to get used to speaking with the immediate denture, and speech may be slightly affected during the adjustment period.
  • Temporary Fit:
Immediate dentures are considered a temporary solution, and a more permanent prosthetic will likely be required after the gums have healed.
  • Risk of Infection:
As with any procedure involving tooth extraction, there is a risk of infection during the healing process. Proper oral hygiene and regular follow-up visits are essential to avoid complications.

How to Choose the Right Doctor and Hospital

When selecting a doctor and hospital for an immediate maxillary denture, it is essential to prioritize experience, expertise, and patient satisfaction. Look for a prosthodontist or oral surgeon who specializes in dentures and tooth replacement procedures. Verify their credentials and ensure they have experience with immediate dentures, as this requires a delicate balance of precision and artistry to achieve an optimal fit and appearance.

The hospital or clinic should be equipped with modern dental technology and follow strict hygiene standards. Check for accreditation from relevant international health authorities, as this ensures that the facility meets global standards for safety and quality of care. Furthermore, consider reading patient reviews and testimonials to gauge the experiences of others who have undergone the same procedure at the facility.

Frequently Asked Questions

What actually happens during hyperstimulation of the ovaries?

The patient will take injectable FSH (follicle stimulating hormone) for eight to eleven days, depending on how long the follicles take to mature. This hormone is produced naturally in a woman’s body causing one egg to develop per cycle. Taking the injectable FSH causes several follicles to develop at once, at approximately the same rate. The development is monitored with vaginal ultrasounds and following the patient’s levels of estradiol and progesterone. FSH brand names include Repronex, Follistim, Menopur, Gonal-F and Bravelle. The patient injects herself daily.

What happens during egg retrieval?

When the follicles have developed enough to be harvested, the patient attends an appointment  where she is anesthetized and prepared for the procedure. Next, the doctor uses an ultrasound probe to guide a needle through the vaginal wall and into the follicle of the ovary. The thin needle draws the follicle fluid, which is then examined by an embryologist to find the eggs. The whole process takes about 20 minutes.

What happens to the eggs?

In the next step, the harvested eggs are then fertilized. If the sperm from the potential father, or in some cases, anonymous donor, has normal functionality, the eggs and sperm are placed together in a dish with a nutrient fluid, then incubated overnight to fertilize normally. If the sperm functionality is suboptimal, an embryologist uses Intracytoplasmic Sperm Injection to inject a single sperm into a single egg with an extremely precise glass needle.  Once fertilization is complete, the embryos are assessed and prepared to be transferred to the patient’s uterus.

How are the embryos transferred back to the uterus?

The doctor and the patient will discuss the number of embryos to be transferred. The number of successfully fertilized eggs usually determines the number of eggs to be placed in the uterus. Embryos are transferred to the uterus with transabdominal ultrasound guidance. This process does not require anesthesia, but it can cause minor cervical or uterine discomfort. Following transfer, the patient is advised to take at least one days bed rest and two or three additional days of rest, then 10 to 12 days later, two pregnancy tests are scheduled to confirm success. Once two positive tests are completed, an obstetrical ultrasound is ordered to show the sac, fetal pole, yolk sac and fetal heart rate.

Embryoscope©

Built into this technology there is a microscope with a powerful camera that allows the uninterrupted monitoring of the embryo during its first hours of life. In this way, we can keep a close eye on the embryo, from the moment when the oocyte is inseminated and begins to divide into smaller and smaller cells, until it can be transferred to the uterus.
