Breast Surgery

Steps Involved in IVF:

Procedure Description

Breast surgery is a specialized medical procedure encompassing various treatments designed to alter, reconstruct, or enhance the breast's appearance and function. It includes aesthetic surgeries like augmentation and reduction, as well as reconstructive surgeries, typically following mastectomy, trauma, or congenital deformities. The primary objective is either cosmetic enhancement or to restore the breast’s appearance and functionality, often significantly impacting a patient’s quality of life and confidence.

Breast augmentation, one of the most commonly performed surgeries, involves enhancing breast size using implants or autologous fat transfer. Breast reduction, on the other hand, reduces the breast's size to alleviate discomfort or achieve a balanced appearance. Reconstructive surgeries usually take place after the removal of breast tissue, commonly due to cancer treatment, and involve techniques like implant-based or flap reconstruction, where tissue from other parts of the body (such as the abdomen) is used to recreate the breast shape.

These surgeries demand extensive preoperative planning and consultation to achieve the desired outcome. Each breast surgery type requires different surgical techniques, materials, and patient considerations. For instance, implant-based procedures vary depending on whether saline or silicone implants are used, and whether they’re placed above or below the chest muscle. Each method has its advantages and specific implications for postoperative recovery, aesthetic results, and long-term outcomes.

Procedure Duration

The time required for breast surgery varies depending on the specific procedure chosen. A typical breast augmentation or reduction surgery may last between 1.5 to 3 hours, while more complex procedures, such as flap reconstruction, can take upwards of 6 to 8 hours. The length of each surgery is largely influenced by the level of complexity, the surgical technique, and any additional steps like fat grafting or reshaping, which may extend the duration.

Following the surgery, patients spend several hours in a recovery room under observation to ensure stability before being discharged. Most breast surgeries are outpatient procedures, meaning patients can often go home the same day, although some cases require overnight hospitalization, especially if complex reconstruction is involved.

Recovery time can also vary significantly. For augmentation and reduction, a recovery period of around 2-4 weeks is typical, with most patients resuming light activities within a week. However, a full recovery, where all swelling and discomfort subsides, may take up to six months. For reconstructive surgeries, particularly flap reconstructions, the healing process can extend over several months, with certain activities restricted to ensure optimal healing. Postoperative care, including the management of drains (if used), incisions, and physical therapy for mobility, forms a crucial part of the recovery process.

Benefits

  • Enhanced Appearance and Confidence: Cosmetic breast surgeries help achieve the desired aesthetic, improving self-esteem and confidence.
  • Pain and Discomfort Relief: Breast reduction alleviates back, neck, and shoulder pain, enhancing the overall physical comfort for patients.
  • Restored Body Image Post-Mastectomy: Reconstructive breast surgery offers emotional and psychological benefits by restoring body image and aiding in recovery after cancer.
  • Minimal Downtime with Outpatient Procedures: Many breast surgeries are outpatient, reducing the need for prolonged hospital stays.
  • Customized Options for Individual Needs: Procedures can be tailored, with choices in implant types, surgical techniques, and outcomes to match each patient’s preferences and medical needs.

Risks & Considerations

  • Infection: Breast surgery, like any surgery, carries a risk of infection, which can be minimized by proper hygiene, prescribed antibiotics, and diligent wound care.
  • Scarring: Surgical incisions can result in scarring, though surgeons often employ techniques to minimize visible scars. Scar management therapies post-surgery also play a role in reducing visibility.
  • Capsular Contracture: In augmentation surgeries, scar tissue may form around the implant, causing discomfort and a hardened feel. Regular check-ups help in identifying this early.
  • Implant Rupture or Deflation: Implants can occasionally rupture or deflate, requiring replacement. This is a risk primarily associated with implant-based augmentations.
  • Anesthesia Risks: General anesthesia used during breast surgeries poses some risks, especially for patients with underlying health conditions. A thorough health assessment is essential to minimize this.

How to Choose the Right Doctor and Hospital

Selecting a qualified doctor and hospital for breast surgery is critical. Start by researching board-certified plastic surgeons specializing in breast procedures, as their credentials ensure they’ve undergone rigorous training and assessment. Look for surgeons with extensive experience and a solid track record in the specific type of breast surgery you’re considering, as well as patient reviews or before-and-after images to verify results.

The hospital or clinic you choose should have a reputation for high standards of hygiene, advanced surgical facilities, and attentive post-operative care. Ideally, choose facilities that support international patients, as these hospitals are more likely to offer the resources, language support, and comprehensive services needed for a smooth, comfortable experience.

Frequently Asked Questions

What actually happens during hyperstimulation of the ovaries?

The patient will take injectable FSH (follicle stimulating hormone) for eight to eleven days, depending on how long the follicles take to mature. This hormone is produced naturally in a woman’s body causing one egg to develop per cycle. Taking the injectable FSH causes several follicles to develop at once, at approximately the same rate. The development is monitored with vaginal ultrasounds and following the patient’s levels of estradiol and progesterone. FSH brand names include Repronex, Follistim, Menopur, Gonal-F and Bravelle. The patient injects herself daily.

What happens during egg retrieval?

When the follicles have developed enough to be harvested, the patient attends an appointment  where she is anesthetized and prepared for the procedure. Next, the doctor uses an ultrasound probe to guide a needle through the vaginal wall and into the follicle of the ovary. The thin needle draws the follicle fluid, which is then examined by an embryologist to find the eggs. The whole process takes about 20 minutes.

What happens to the eggs?

In the next step, the harvested eggs are then fertilized. If the sperm from the potential father, or in some cases, anonymous donor, has normal functionality, the eggs and sperm are placed together in a dish with a nutrient fluid, then incubated overnight to fertilize normally. If the sperm functionality is suboptimal, an embryologist uses Intracytoplasmic Sperm Injection to inject a single sperm into a single egg with an extremely precise glass needle.  Once fertilization is complete, the embryos are assessed and prepared to be transferred to the patient’s uterus.

How are the embryos transferred back to the uterus?

The doctor and the patient will discuss the number of embryos to be transferred. The number of successfully fertilized eggs usually determines the number of eggs to be placed in the uterus. Embryos are transferred to the uterus with transabdominal ultrasound guidance. This process does not require anesthesia, but it can cause minor cervical or uterine discomfort. Following transfer, the patient is advised to take at least one days bed rest and two or three additional days of rest, then 10 to 12 days later, two pregnancy tests are scheduled to confirm success. Once two positive tests are completed, an obstetrical ultrasound is ordered to show the sac, fetal pole, yolk sac and fetal heart rate.

Embryoscope©

Built into this technology there is a microscope with a powerful camera that allows the uninterrupted monitoring of the embryo during its first hours of life. In this way, we can keep a close eye on the embryo, from the moment when the oocyte is inseminated and begins to divide into smaller and smaller cells, until it can be transferred to the uterus.
