Total Abdominal Hysterectomy

Steps Involved in IVF:

Procedure Description

A total abdominal hysterectomy (TAH) is a surgical procedure in which the uterus is removed through an incision in the lower abdomen. Often performed to treat uterine fibroids, endometriosis, chronic pelvic pain, or certain cancers, TAH is a comprehensive approach that enables surgeons to address both the uterus and surrounding tissues if necessary. The procedure may also involve removing the cervix and, depending on the patient's needs, the ovaries and fallopian tubes. Since it is considered a major surgery, TAH requires skilled surgical expertise and a well-prepared patient care team.

During TAH, a horizontal or vertical incision is made in the abdominal area, giving the surgeon full access to the uterus and surrounding tissues. The surgical approach allows for a more thorough examination of the pelvic region and helps ensure all abnormal tissue is removed. While minimally invasive options like laparoscopic or vaginal hysterectomy exist, TAH is often recommended for patients with complex conditions or advanced disease stages where greater precision is necessary.

The surgery typically requires general anesthesia, and the procedure itself can last anywhere from one to three hours, depending on factors like patient health and surgical complexity. After removal of the uterus (and other reproductive organs if necessary), the incision is closed with sutures or staples, followed by a postoperative observation period. While the procedure's ultimate purpose is to alleviate symptoms or prevent disease progression, it also involves a recovery phase that patients should prepare for.

Procedure Duration

The total abdominal hysterectomy is generally completed within one to three hours, although more complex cases may require additional time. The surgical procedure is divided into stages, each essential for the procedure's success and patient safety.

First, the patient is prepared in the preoperative room, where anesthesia is administered. Once the patient is under anesthesia, the surgical team proceeds with the incision and locates the uterus. Next, the uterus and any other tissues needing removal are carefully detached from surrounding structures. This step is crucial, as it involves close attention to nearby organs and blood vessels, ensuring that no injury occurs during the procedure.

Following the surgery, patients are monitored in the postoperative recovery room. This observation period usually spans two to three hours, depending on the individual’s response to anesthesia and initial recovery. While hospital stays vary based on the patient’s health and surgical details, most patients can expect a stay of one to three days to ensure adequate initial healing and pain management. Full recovery from a TAH can take six to eight weeks, with the first few weeks focused on avoiding strenuous activities and ensuring the body heals without strain.

Benefits

  • Comprehensive Treatment: TAH can treat a wide range of conditions, including fibroids, endometriosis, chronic pelvic pain, and certain cancers.
  • Long-Term Relief: Many patients experience permanent relief from symptoms like heavy bleeding, severe cramping, and pelvic pain after a TAH.
  • Improved Quality of Life: For those with chronic pelvic conditions, TAH often leads to a significant improvement in day-to-day activities and overall quality of life.
  • Cancer Prevention: TAH is sometimes recommended as a preventive measure for patients at high risk of gynecologic cancers, particularly when other treatments are insufficient.
  • Reduction in Hormonal Disruptions: In cases where only the uterus is removed, the hormonal impact is less severe than a complete removal of reproductive organs, preserving ovarian function if retained.

Risks & Considerations

  • Surgical Risks
As with any major surgery, there is a risk of complications such as infection, bleeding, and blood clots. Patients should discuss these risks with their healthcare provider before the procedure to understand preventive measures.
  • Recovery Challenges
TAH requires a significant recovery period of up to eight weeks. During this time, patients may experience discomfort, limited mobility, and emotional adjustments. Adequate support at home and time off from work are essential for a smooth recovery.
  • Hormonal Changes
If the ovaries are removed during TAH, patients will experience menopausal symptoms due to a decrease in estrogen. This change can be managed through hormone replacement therapy (HRT) if recommended by the healthcare provider.
  • Impact on Sexual Health
Some patients may experience changes in sexual function following TAH. While many individuals report improved symptoms post-procedure, others may have a reduced libido or discomfort during intercourse, especially if the cervix is removed.
  • Psychological Adjustment
TAH can be an emotional experience, especially for those of reproductive age. Psychological support and counseling can help patients cope with these changes and adjust to life post-surgery.

How to Choose the Right Doctor and Hospital

Choosing the right doctor and hospital for a total abdominal hysterectomy is essential for a successful outcome. Patients should seek a board-certified gynecological surgeon with extensive experience in performing TAH. A surgeon specializing in minimally invasive gynecologic procedures may also be beneficial, as they are typically well-versed in complex surgeries. Seeking reviews, consulting with former patients, and verifying the surgeon’s credentials can help ensure high-quality care.

In terms of hospital selection, look for facilities with internationally recognized certifications and strong reputations in women’s healthcare. Many reputable hospitals offer comprehensive preoperative and postoperative services, which can be especially beneficial for medical tourists. Language support, dedicated patient coordinators, and personalized care options also play a critical role in providing a positive experience for patients traveling for medical care.

Frequently Asked Questions

What actually happens during hyperstimulation of the ovaries?

The patient will take injectable FSH (follicle stimulating hormone) for eight to eleven days, depending on how long the follicles take to mature. This hormone is produced naturally in a woman’s body causing one egg to develop per cycle. Taking the injectable FSH causes several follicles to develop at once, at approximately the same rate. The development is monitored with vaginal ultrasounds and following the patient’s levels of estradiol and progesterone. FSH brand names include Repronex, Follistim, Menopur, Gonal-F and Bravelle. The patient injects herself daily.

What happens during egg retrieval?

When the follicles have developed enough to be harvested, the patient attends an appointment  where she is anesthetized and prepared for the procedure. Next, the doctor uses an ultrasound probe to guide a needle through the vaginal wall and into the follicle of the ovary. The thin needle draws the follicle fluid, which is then examined by an embryologist to find the eggs. The whole process takes about 20 minutes.

What happens to the eggs?

In the next step, the harvested eggs are then fertilized. If the sperm from the potential father, or in some cases, anonymous donor, has normal functionality, the eggs and sperm are placed together in a dish with a nutrient fluid, then incubated overnight to fertilize normally. If the sperm functionality is suboptimal, an embryologist uses Intracytoplasmic Sperm Injection to inject a single sperm into a single egg with an extremely precise glass needle.  Once fertilization is complete, the embryos are assessed and prepared to be transferred to the patient’s uterus.

How are the embryos transferred back to the uterus?

The doctor and the patient will discuss the number of embryos to be transferred. The number of successfully fertilized eggs usually determines the number of eggs to be placed in the uterus. Embryos are transferred to the uterus with transabdominal ultrasound guidance. This process does not require anesthesia, but it can cause minor cervical or uterine discomfort. Following transfer, the patient is advised to take at least one days bed rest and two or three additional days of rest, then 10 to 12 days later, two pregnancy tests are scheduled to confirm success. Once two positive tests are completed, an obstetrical ultrasound is ordered to show the sac, fetal pole, yolk sac and fetal heart rate.

Embryoscope©

Built into this technology there is a microscope with a powerful camera that allows the uninterrupted monitoring of the embryo during its first hours of life. In this way, we can keep a close eye on the embryo, from the moment when the oocyte is inseminated and begins to divide into smaller and smaller cells, until it can be transferred to the uterus.
