Primary Rhinoplasty

Steps Involved in IVF:

Procedure Description:

Primary rhinoplasty, often referred to simply as a "nose job," is a surgical procedure designed to alter the shape and functionality of the nose. The term "primary" signifies that it is the first rhinoplasty surgery performed on the nose, as opposed to a revision rhinoplasty, which is performed to correct or enhance the outcome of a previous rhinoplasty. The surgery is widely sought after for both aesthetic and functional reasons, addressing concerns related to the nose’s appearance, such as size, shape, or symmetry, or correcting breathing issues caused by structural abnormalities.

The procedure begins with a consultation where the surgeon assesses the patient’s nasal structure, discusses expectations, and develops a personalized surgical plan. Primary rhinoplasty is performed under general anesthesia and can involve various techniques depending on the desired outcome. There are two main surgical approaches: the open technique, which involves an incision made across the columella (the tissue between the nostrils), and the closed technique, where incisions are made inside the nostrils. The open technique provides better visibility for more complex reshaping, while the closed technique is less invasive and leaves no external scars.

During the surgery, the surgeon may reduce or augment nasal structures using cartilage grafts or reshaping techniques. In cases where nasal breathing is impaired, structural corrections such as septoplasty (realigning a deviated septum) are often performed concurrently. The goal is to create a harmonious and natural-looking nose that fits the patient’s facial features while preserving or enhancing nasal function. The duration of the surgery and techniques used vary depending on the individual’s needs and anatomy.

Procedure Duration:

Primary rhinoplasty typically lasts between 1.5 to 3 hours, depending on the complexity of the case and the surgical techniques involved. The procedure begins with the administration of general anesthesia to ensure the patient’s comfort throughout the surgery. Once the anesthesia takes effect, the surgeon performs the necessary incisions and reshapes the nasal bones, cartilage, and soft tissue to achieve the desired appearance or functional correction.

After surgery, patients enter the recovery phase. Immediate recovery from rhinoplasty usually involves staying in the recovery room for a few hours under medical supervision. Once discharged, the initial recovery period lasts about 1 to 2 weeks, during which the patient will experience swelling, bruising around the eyes, and possible nasal congestion. A nasal splint is often applied to support the new nasal structure during this time, and it is usually removed after a week.

Full recovery from primary rhinoplasty takes several months. While most swelling and bruising subside within a couple of weeks, subtle changes in nasal shape continue to refine over time as the swelling further diminishes. Most patients can resume normal activities within 10 to 14 days, but strenuous activities should be avoided for several weeks. The final results of the surgery become more apparent after 6 to 12 months when the nose has fully settled into its new shape.

Benefits of Primary Rhinoplasty:

  • Improved nasal aesthetics: Enhances the symmetry, size, and shape of the nose to better align with the patient’s facial features.
  • Correction of breathing difficulties: Resolves structural issues like a deviated septum, improving nasal airflow and breathing.
  • Boost in self-confidence: Many patients experience increased self-esteem after achieving their desired facial appearance.
  • Customizable results: The surgery can be tailored to address specific concerns, whether cosmetic, functional, or both.
  • Permanent improvement: The results of primary rhinoplasty are long-lasting, offering permanent improvements when performed successfully.

Additionally, Thailand’s well-developed tourism industry makes it an attractive destination for post-surgical recovery.

Risks & Considerations:

  • Swelling and bruising: These are common side effects of rhinoplasty, and while temporary, they may take several weeks to subside.
  • Infection: As with any surgical procedure, there is a risk of infection. Following post-operative care instructions minimizes this risk.
  • Nasal obstruction: Some patients may experience temporary difficulty breathing through the nose as a result of swelling or internal healing.
  • Unsatisfactory results: There is a possibility that the outcome may not meet the patient’s expectations, requiring revision surgery.
  • Scarring: Though minimal, there is a risk of visible scarring, especially with the open rhinoplasty technique.

How to Choose the Right Doctor and Hospital:

When selecting a surgeon for primary rhinoplasty, it is essential to focus on experience and specialization. A qualified facial plastic surgeon who regularly performs rhinoplasties is likely to deliver better results due to their deep understanding of nasal anatomy and aesthetic principles. Look for board-certified surgeons who have a track record of successful rhinoplasty outcomes, and request to see before-and-after photos of previous patients to gauge their expertise.

In addition to selecting the right surgeon, choosing the appropriate hospital or surgical facility is equally crucial. Opt for accredited medical centers that meet international standards for hygiene, patient care, and safety protocols. Facilities that specialize in cosmetic or plastic surgery often provide a more tailored experience and have teams dedicated to post-operative care, which can significantly enhance the overall outcome of the surgery. Ensure that the hospital offers comprehensive pre- and post-surgery care to support a smooth recovery process.

Frequently Asked Questions

What actually happens during hyperstimulation of the ovaries?

The patient will take injectable FSH (follicle stimulating hormone) for eight to eleven days, depending on how long the follicles take to mature. This hormone is produced naturally in a woman’s body causing one egg to develop per cycle. Taking the injectable FSH causes several follicles to develop at once, at approximately the same rate. The development is monitored with vaginal ultrasounds and following the patient’s levels of estradiol and progesterone. FSH brand names include Repronex, Follistim, Menopur, Gonal-F and Bravelle. The patient injects herself daily.

What happens during egg retrieval?

When the follicles have developed enough to be harvested, the patient attends an appointment  where she is anesthetized and prepared for the procedure. Next, the doctor uses an ultrasound probe to guide a needle through the vaginal wall and into the follicle of the ovary. The thin needle draws the follicle fluid, which is then examined by an embryologist to find the eggs. The whole process takes about 20 minutes.

What happens to the eggs?

In the next step, the harvested eggs are then fertilized. If the sperm from the potential father, or in some cases, anonymous donor, has normal functionality, the eggs and sperm are placed together in a dish with a nutrient fluid, then incubated overnight to fertilize normally. If the sperm functionality is suboptimal, an embryologist uses Intracytoplasmic Sperm Injection to inject a single sperm into a single egg with an extremely precise glass needle.  Once fertilization is complete, the embryos are assessed and prepared to be transferred to the patient’s uterus.

How are the embryos transferred back to the uterus?

The doctor and the patient will discuss the number of embryos to be transferred. The number of successfully fertilized eggs usually determines the number of eggs to be placed in the uterus. Embryos are transferred to the uterus with transabdominal ultrasound guidance. This process does not require anesthesia, but it can cause minor cervical or uterine discomfort. Following transfer, the patient is advised to take at least one days bed rest and two or three additional days of rest, then 10 to 12 days later, two pregnancy tests are scheduled to confirm success. Once two positive tests are completed, an obstetrical ultrasound is ordered to show the sac, fetal pole, yolk sac and fetal heart rate.

Embryoscope©

Built into this technology there is a microscope with a powerful camera that allows the uninterrupted monitoring of the embryo during its first hours of life. In this way, we can keep a close eye on the embryo, from the moment when the oocyte is inseminated and begins to divide into smaller and smaller cells, until it can be transferred to the uterus.
