Physical Therapy

Steps Involved in IVF:

Procedure Description

Physical therapy is a medical treatment focusing on restoring movement, function, and improving quality of life for those suffering from physical impairments or disabilities. Often recommended as a part of a rehabilitation plan, physical therapy encompasses a broad range of therapeutic exercises, manual interventions, and educational advice aimed at enhancing physical strength, flexibility, and endurance. The goal is to help patients regain their functional independence and reduce pain without relying heavily on medications or invasive procedures.

Physical therapists use a variety of techniques and tools tailored to the patient's individual needs. These may include exercises to improve mobility and strength, balance training, posture correction, manual therapies such as soft tissue massage or joint mobilization, and the use of modalities like heat, ice, electrical stimulation, and ultrasound. Treatment is often customized based on the patient’s unique medical history, current physical condition, and therapeutic goals.

The process typically begins with an in-depth evaluation, where the therapist assesses the patient's range of motion, strength, posture, and functional limitations. From this assessment, an individualized treatment plan is developed, setting out clear objectives and timelines for achieving specific rehabilitation milestones.

Procedure Duration

The duration of physical therapy varies widely depending on the underlying condition being treated, the severity of symptoms, and the patient's progress. A typical session lasts between 45 to 60 minutes, and most patients attend therapy two to three times a week. The overall length of a physical therapy program may range from a few weeks for minor injuries to several months or even longer for chronic conditions or extensive post-surgical rehabilitation.

Therapy is often broken down into different phases. The initial phase may focus on pain management and early mobility exercises, while the intermediate phase aims to restore normal movement patterns, improve strength, and reduce functional impairments. The final phase often involves advanced exercises to restore complete function and prevent recurrence, often transitioning the patient to a home-based exercise routine for ongoing self-management.

Recovery is a gradual process, and it is common for patients to notice small improvements week by week. A key aspect of physical therapy is its adaptability, with therapists continuously modifying the treatment plan based on the patient's progress and response to therapy. Adherence to prescribed exercises and instructions, both within sessions and at home, is essential for maximizing the effectiveness of the treatment.

Risks & Considerations

  • Delayed Recovery: Some patients may experience slower-than-expected progress or incomplete recovery if physical therapy is not properly followed or individualized.
  • Pain & Discomfort: While physical therapy aims to reduce pain, certain exercises and techniques may cause temporary discomfort or exacerbate symptoms.
  • Improper Technique: Incorrect performance of exercises can lead to further injury, underscoring the importance of a therapist’s guidance and adherence to instructions.
  • Overuse Injuries: If exercises are performed too frequently or aggressively without proper guidance, there is a risk of developing overuse injuries.
  • Dependency on Therapy: In some cases, patients may become overly reliant on the therapist, which can hinder the development of self-management skills necessary for long-term maintenance.

How to Choose the Right Doctor and Hospital

Selecting the right doctor and hospital for physical therapy is crucial for successful treatment and recovery. Look for therapists who are licensed and hold relevant certifications from recognized institutions, along with significant experience in treating the specific condition or injury you are dealing with. It is also wise to choose a therapist who uses evidence-based practices and stays current with the latest developments in physical therapy.

When considering a hospital or rehabilitation center, ensure they have a strong track record in rehabilitation services, with access to advanced facilities, diverse therapeutic modalities, and comprehensive programs tailored to individual patient needs. Additionally, the hospital should have a multidisciplinary team approach, including access to other specialists like orthopedists, neurologists, or pain management experts, as this can significantly enhance the quality of care and recovery outcomes.

Frequently Asked Questions

What actually happens during hyperstimulation of the ovaries?

The patient will take injectable FSH (follicle stimulating hormone) for eight to eleven days, depending on how long the follicles take to mature. This hormone is produced naturally in a woman’s body causing one egg to develop per cycle. Taking the injectable FSH causes several follicles to develop at once, at approximately the same rate. The development is monitored with vaginal ultrasounds and following the patient’s levels of estradiol and progesterone. FSH brand names include Repronex, Follistim, Menopur, Gonal-F and Bravelle. The patient injects herself daily.

What happens during egg retrieval?

When the follicles have developed enough to be harvested, the patient attends an appointment  where she is anesthetized and prepared for the procedure. Next, the doctor uses an ultrasound probe to guide a needle through the vaginal wall and into the follicle of the ovary. The thin needle draws the follicle fluid, which is then examined by an embryologist to find the eggs. The whole process takes about 20 minutes.

What happens to the eggs?

In the next step, the harvested eggs are then fertilized. If the sperm from the potential father, or in some cases, anonymous donor, has normal functionality, the eggs and sperm are placed together in a dish with a nutrient fluid, then incubated overnight to fertilize normally. If the sperm functionality is suboptimal, an embryologist uses Intracytoplasmic Sperm Injection to inject a single sperm into a single egg with an extremely precise glass needle.  Once fertilization is complete, the embryos are assessed and prepared to be transferred to the patient’s uterus.

How are the embryos transferred back to the uterus?

The doctor and the patient will discuss the number of embryos to be transferred. The number of successfully fertilized eggs usually determines the number of eggs to be placed in the uterus. Embryos are transferred to the uterus with transabdominal ultrasound guidance. This process does not require anesthesia, but it can cause minor cervical or uterine discomfort. Following transfer, the patient is advised to take at least one days bed rest and two or three additional days of rest, then 10 to 12 days later, two pregnancy tests are scheduled to confirm success. Once two positive tests are completed, an obstetrical ultrasound is ordered to show the sac, fetal pole, yolk sac and fetal heart rate.

Embryoscope©

Built into this technology there is a microscope with a powerful camera that allows the uninterrupted monitoring of the embryo during its first hours of life. In this way, we can keep a close eye on the embryo, from the moment when the oocyte is inseminated and begins to divide into smaller and smaller cells, until it can be transferred to the uterus.
