Phototherapy

Steps Involved in IVF:

Procedure Description:

Phototherapy, commonly referred to as light therapy, is a non-invasive medical treatment that uses specific wavelengths of light to treat various skin and health conditions. It is a well-established treatment for skin disorders like psoriasis, eczema, and vitiligo. During phototherapy, a patient’s skin is exposed to controlled doses of ultraviolet (UV) light under medical supervision. The light therapy can involve either ultraviolet A (UVA) or ultraviolet B (UVB) light, or a combination of both. Each type of light has unique properties that target the skin in specific ways, helping to reduce inflammation, promote skin healing, and manage symptoms.

This treatment involves targeted exposure, where affected areas receive UV light exposure at a dosage precisely measured for therapeutic impact. Typically, phototherapy sessions are performed in a healthcare setting using specialized equipment designed to emit UV rays safely. The two most common types of phototherapy are broadband UVB and narrowband UVB. Narrowband UVB, in particular, is highly effective in treating many skin conditions and is associated with fewer side effects compared to older broadband UVB methods.

Beyond skin disorders, phototherapy has applications in neonatal care, particularly in treating newborn jaundice. In this case, blue light therapy helps to break down excess bilirubin in a newborn’s blood, aiding in its safe elimination. With advancements in medical technology, phototherapy has become more precise, ensuring optimal therapeutic outcomes while minimizing exposure risks.

Procedure Duration:

The duration of phototherapy varies depending on the condition being treated, the severity of the case, and the individual’s response to the treatment. Each session typically lasts anywhere from a few minutes to half an hour. For skin conditions like psoriasis and eczema, patients may undergo phototherapy two to three times a week, with the total duration of treatment spanning several weeks to months. Treatment schedules are adjusted as the condition improves, reducing the frequency of sessions to maintain results over time.

The phases of treatment can also vary. Initial treatment may require higher frequency to reduce symptoms quickly. Once improvement is noted, the frequency of sessions is reduced in what’s known as a “maintenance phase.” In neonatal jaundice cases, light therapy may be needed for several hours a day, sometimes continuously, until bilirubin levels return to safe ranges. The exact timing and duration of each phototherapy session depend on the equipment used and the healthcare provider's specific treatment plan.

Recovery from phototherapy is typically immediate, as no incisions or invasive techniques are involved. However, some patients may experience temporary skin redness, dryness, or mild itching following treatment. Skin hydration is often recommended post-treatment, and patients are advised to avoid additional sun exposure to prevent overexposure to UV rays.

Benefits:

  • Effective Treatment for Skin Conditions: Phototherapy has proven effective in managing conditions like psoriasis, eczema, vitiligo, and other inflammatory skin disorders.
  • Non-Invasive Procedure: The procedure does not involve surgery, incisions, or anesthesia, reducing recovery time and minimizing risk.
  • Customizable Dosages: Phototherapy sessions are tailored to meet individual needs, with precise UV exposure levels adjusted to achieve optimal therapeutic effects.
  • Minimal Side Effects: Narrowband UVB therapy, in particular, has fewer side effects compared to traditional treatments, making it a safer option for long-term use.
  • Beneficial for Neonatal Jaundice: Blue light phototherapy is a critical treatment for newborns with jaundice, offering a safe way to lower bilirubin levels.

Risks & Considerations:

  • Skin Sensitivity: Some patients may experience temporary skin redness, itching, or dryness post-treatment. It is essential to moisturize the skin and follow aftercare instructions to alleviate these symptoms.
  • Risk of Overexposure: Although the treatment is carefully controlled, excessive exposure to UV light can increase the risk of skin cancer over time. Patients are advised to minimize additional UV exposure outside of treatment sessions.
  • Photosensitivity: Certain medications can increase sensitivity to UV light, which may lead to adverse skin reactions. Patients should inform their healthcare provider of any medications or supplements they are taking.
  • Eye Protection: UV light can damage the eyes if not properly protected during treatment. Patients should wear protective eyewear provided by the healthcare facility to prevent UV-related eye injuries.
  • Contraindications for Specific Skin Conditions: Phototherapy is not suitable for everyone. Those with a history of skin cancer, certain autoimmune diseases, or hypersensitivity to light should consult their doctor to assess the risks.

How to Choose the Right Doctor and Hospital:

When selecting a doctor and hospital for phototherapy, it’s essential to verify the facility’s experience with this specific treatment. Seek medical centers that specialize in dermatology or neonatal care, depending on the condition. It’s beneficial to choose a facility with modern equipment and a well-established phototherapy unit, as this indicates adherence to the latest safety standards and treatment efficacy.

Additionally, look for licensed dermatologists or neonatologists with a proven track record in phototherapy. Reading reviews, consulting with medical tourism agencies, and asking for pre-treatment consultations can provide valuable insight into the doctor’s expertise and the facility’s quality of care.

Frequently Asked Questions

What actually happens during hyperstimulation of the ovaries?

The patient will take injectable FSH (follicle stimulating hormone) for eight to eleven days, depending on how long the follicles take to mature. This hormone is produced naturally in a woman’s body causing one egg to develop per cycle. Taking the injectable FSH causes several follicles to develop at once, at approximately the same rate. The development is monitored with vaginal ultrasounds and following the patient’s levels of estradiol and progesterone. FSH brand names include Repronex, Follistim, Menopur, Gonal-F and Bravelle. The patient injects herself daily.

What happens during egg retrieval?

When the follicles have developed enough to be harvested, the patient attends an appointment  where she is anesthetized and prepared for the procedure. Next, the doctor uses an ultrasound probe to guide a needle through the vaginal wall and into the follicle of the ovary. The thin needle draws the follicle fluid, which is then examined by an embryologist to find the eggs. The whole process takes about 20 minutes.

What happens to the eggs?

In the next step, the harvested eggs are then fertilized. If the sperm from the potential father, or in some cases, anonymous donor, has normal functionality, the eggs and sperm are placed together in a dish with a nutrient fluid, then incubated overnight to fertilize normally. If the sperm functionality is suboptimal, an embryologist uses Intracytoplasmic Sperm Injection to inject a single sperm into a single egg with an extremely precise glass needle.  Once fertilization is complete, the embryos are assessed and prepared to be transferred to the patient’s uterus.

How are the embryos transferred back to the uterus?

The doctor and the patient will discuss the number of embryos to be transferred. The number of successfully fertilized eggs usually determines the number of eggs to be placed in the uterus. Embryos are transferred to the uterus with transabdominal ultrasound guidance. This process does not require anesthesia, but it can cause minor cervical or uterine discomfort. Following transfer, the patient is advised to take at least one days bed rest and two or three additional days of rest, then 10 to 12 days later, two pregnancy tests are scheduled to confirm success. Once two positive tests are completed, an obstetrical ultrasound is ordered to show the sac, fetal pole, yolk sac and fetal heart rate.

Embryoscope©

Built into this technology there is a microscope with a powerful camera that allows the uninterrupted monitoring of the embryo during its first hours of life. In this way, we can keep a close eye on the embryo, from the moment when the oocyte is inseminated and begins to divide into smaller and smaller cells, until it can be transferred to the uterus.
