Obstetrics consultation

Steps Involved in IVF:

Procedure Description

Obstetrics consultation is a vital step for pregnant women seeking comprehensive care throughout their pregnancy journey. This consultation focuses on maternal health, fetal development, and ensuring that both mother and baby receive optimal care. Typically, an obstetrician, a medical professional specializing in pregnancy, childbirth, and postpartum care, leads the consultation. These sessions involve thorough discussions and evaluations of a mother’s medical history, lifestyle, prenatal symptoms, and potential risk factors.

During the initial obstetrics consultation, a detailed review of the mother’s health history and current lifestyle is conducted to identify any factors that may affect the pregnancy. Discussions may encompass the woman’s age, any existing health conditions, previous pregnancies, and family health history. The obstetrician also addresses lifestyle factors such as diet, exercise habits, and prenatal vitamins to ensure optimal fetal health.

The obstetrics consultation is also an opportunity for expectant mothers to ask questions and voice concerns. Topics may include birth plans, pain management options, what to expect in each trimester, and ways to mitigate common pregnancy symptoms such as nausea, fatigue, and back pain. These consultations are an integral part of prenatal care, focusing on the prevention of complications, early detection of any health issues, and preparation for delivery.

Procedure Duration

The obstetrics consultation process usually spans the entire duration of the pregnancy. It starts with an initial visit, often within the first 8 to 12 weeks of gestation, where a comprehensive assessment of the mother's health and fetal well-being is performed. This first visit is typically more in-depth than follow-up visits, lasting anywhere between 60 to 90 minutes.

Subsequent consultations are shorter, generally lasting around 20 to 30 minutes. These are scheduled monthly until the pregnancy reaches the 28-week mark, biweekly until 36 weeks, and then weekly until delivery. The length of each session may vary depending on the progression of the pregnancy, any complications or concerns that arise, and specific needs of the mother.

The consultation process extends beyond the delivery of the baby. Postpartum visits are crucial to monitor the mother's recovery, emotional well-being, and discuss newborn care. A typical postpartum consultation occurs around six weeks after childbirth. Recovery times for mothers vary, with vaginal deliveries generally requiring shorter recovery periods compared to cesarean sections.

Risks & Considerations

  • Language Barriers: For international consultations, language differences can pose communication challenges. It is essential to ensure the availability of interpreters or choose a provider fluent in the patient's language.
  • Different Medical Protocols: Medical standards and practices can vary between countries. Expectant mothers must understand the medical protocols followed in the destination country to ensure they align with their needs and preferences.
  • Insurance Coverage: Not all insurance plans cover obstetrics consultation abroad. Verifying what is included in the insurance policy beforehand can help avoid unexpected costs.
  • Post-Consultation Follow-up: If receiving care abroad, coordinating post-consultation follow-ups and ongoing care with local healthcare providers upon return is crucial for continuous support.
  • Travel During Pregnancy: Travel, especially long-haul flights, can pose risks during pregnancy. It’s important to consider the optimal time for travel, typically the second trimester, and follow any medical advice related to flying while pregnant.

How to Choose the Right Doctor and Hospital

Selecting the right obstetrician and healthcare facility is pivotal to a safe and healthy pregnancy. Begin by researching the credentials and experience of potential doctors, ensuring they are board-certified in obstetrics and have positive patient reviews. It is also important to find a doctor who communicates clearly and empathetically, as the consultation process involves open discussions about health and personal concerns.

When choosing a hospital or clinic, prioritize facilities with accreditation from reputable international healthcare organizations. This ensures adherence to high standards of safety, hygiene, and patient care. Look for hospitals offering comprehensive maternity services, including prenatal consultations, delivery options, and postpartum care, to provide a seamless experience throughout the pregnancy journey.

Frequently Asked Questions

What actually happens during hyperstimulation of the ovaries?

The patient will take injectable FSH (follicle stimulating hormone) for eight to eleven days, depending on how long the follicles take to mature. This hormone is produced naturally in a woman’s body causing one egg to develop per cycle. Taking the injectable FSH causes several follicles to develop at once, at approximately the same rate. The development is monitored with vaginal ultrasounds and following the patient’s levels of estradiol and progesterone. FSH brand names include Repronex, Follistim, Menopur, Gonal-F and Bravelle. The patient injects herself daily.

What happens during egg retrieval?

When the follicles have developed enough to be harvested, the patient attends an appointment  where she is anesthetized and prepared for the procedure. Next, the doctor uses an ultrasound probe to guide a needle through the vaginal wall and into the follicle of the ovary. The thin needle draws the follicle fluid, which is then examined by an embryologist to find the eggs. The whole process takes about 20 minutes.

What happens to the eggs?

In the next step, the harvested eggs are then fertilized. If the sperm from the potential father, or in some cases, anonymous donor, has normal functionality, the eggs and sperm are placed together in a dish with a nutrient fluid, then incubated overnight to fertilize normally. If the sperm functionality is suboptimal, an embryologist uses Intracytoplasmic Sperm Injection to inject a single sperm into a single egg with an extremely precise glass needle.  Once fertilization is complete, the embryos are assessed and prepared to be transferred to the patient’s uterus.

How are the embryos transferred back to the uterus?

The doctor and the patient will discuss the number of embryos to be transferred. The number of successfully fertilized eggs usually determines the number of eggs to be placed in the uterus. Embryos are transferred to the uterus with transabdominal ultrasound guidance. This process does not require anesthesia, but it can cause minor cervical or uterine discomfort. Following transfer, the patient is advised to take at least one days bed rest and two or three additional days of rest, then 10 to 12 days later, two pregnancy tests are scheduled to confirm success. Once two positive tests are completed, an obstetrical ultrasound is ordered to show the sac, fetal pole, yolk sac and fetal heart rate.

Embryoscope©

Built into this technology there is a microscope with a powerful camera that allows the uninterrupted monitoring of the embryo during its first hours of life. In this way, we can keep a close eye on the embryo, from the moment when the oocyte is inseminated and begins to divide into smaller and smaller cells, until it can be transferred to the uterus.
