Lipostructure

Steps Involved in IVF:

Procedure Description

Lipostructure, also known as autologous fat grafting or lipofilling, is an innovative cosmetic procedure aimed at restoring volume and contour to various parts of the body. This technique involves extracting the patient’s own fat from donor areas, such as the abdomen or thighs, purifying it, and then strategically injecting it into targeted areas. Often chosen for facial rejuvenation, it is also effective for breast enhancement, buttock contouring, and reconstructive purposes. By utilizing the patient's own cells, lipostructure minimizes the risk of allergic reactions and promotes a more natural appearance compared to synthetic fillers.

During the lipostructure procedure, the fat is harvested through liposuction, a minimally invasive technique that gently suctions fat cells. Once collected, the fat undergoes a purification process to remove impurities and excess fluids, leaving only healthy fat cells suitable for injection. The purified fat is then injected meticulously into the recipient site in a series of fine layers, ensuring even distribution and optimal integration with surrounding tissues. This layering technique is essential for achieving a smooth, natural-looking result and enhances the longevity of the grafted fat.

Lipostructure is a popular choice due to its dual benefit: patients can achieve both contouring of the donor area and enhanced volume in the treated site. Furthermore, the procedure encourages tissue regeneration as fat cells bring with them stem cells that promote healing and rejuvenation. However, patients must be prepared for a careful assessment to determine suitability, as factors like overall health, lifestyle, and desired outcomes will affect candidacy for lipostructure.

Procedure Duration

The lipostructure procedure can vary in length depending on the area being treated and the amount of fat being transferred. On average, a typical session lasts between two to four hours, including the harvesting, purification, and grafting phases. Each step is crucial, as precision and time are necessary to ensure the successful transfer and integration of fat cells.

The process begins with liposuction, which generally takes about 30 minutes to an hour, depending on the volume needed. After extraction, the fat undergoes purification in a sterile environment, a step that can take around 30 minutes to complete. This purified fat is then ready for reinjection, which may take up to an hour or more, depending on the complexity of the area and the aesthetic goals.

Recovery from lipostructure varies by individual, but most patients experience minor bruising and swelling in both the donor and recipient areas, which subsides within a week or two. Full results can be seen in several months as the transferred fat settles and integrates with the surrounding tissues. Patients typically resume light activities within a few days and more strenuous activities after two to three weeks. It’s important to follow post-procedure guidelines closely, as these will significantly impact the longevity and quality of the results.

Benefits of Lipostructure

  • Natural Appearance: Using the patient’s own fat produces a more natural and harmonious aesthetic compared to synthetic fillers.
  • Dual Benefit of Contouring: Patients can enhance volume in the desired area while also achieving contouring in the donor area.
  • Reduced Allergy Risks: The use of autologous fat minimizes the likelihood of allergic reactions or rejection.
  • Long-Lasting Results: When the grafted fat is well-integrated, it provides enduring results, especially with proper post-procedure care.
  • Enhanced Rejuvenation: Stem cells in fat promote tissue repair and rejuvenation, resulting in a fresher, younger appearance.

Risks & Considerations

  • Fat Resorption: Not all transferred fat will survive; some may be reabsorbed by the body, affecting the final volume.
  • Uneven Results: Inexperienced providers or inadequate techniques can lead to asymmetrical or uneven results.
  • Infection Risks: As with any surgical procedure, there is a risk of infection, especially if post-operative care is not strictly followed.
  • Bruising and Swelling: Both the donor and recipient areas may experience bruising and swelling, which may take time to subside.
  • Necrosis: Fat necrosis, or the death of some transferred fat cells, is a rare but possible risk that can result in lumps or irregular texture.

How to Choose the Right Doctor and Hospital

Selecting a qualified provider for lipostructure is essential to ensure both safety and satisfaction with the outcome. Look for a board-certified plastic surgeon with extensive experience in fat grafting procedures. Research their previous work, paying attention to patient reviews, before-and-after photos, and the surgeon’s specialty in lipostructure or similar procedures.

When considering the hospital or clinic, prioritize facilities with reputable accreditation and high standards of hygiene, safety, and patient care. Ask about the specific protocols they follow for fat handling and post-operative care, as these can directly impact the success of the procedure.

Frequently Asked Questions

What actually happens during hyperstimulation of the ovaries?

The patient will take injectable FSH (follicle stimulating hormone) for eight to eleven days, depending on how long the follicles take to mature. This hormone is produced naturally in a woman’s body causing one egg to develop per cycle. Taking the injectable FSH causes several follicles to develop at once, at approximately the same rate. The development is monitored with vaginal ultrasounds and following the patient’s levels of estradiol and progesterone. FSH brand names include Repronex, Follistim, Menopur, Gonal-F and Bravelle. The patient injects herself daily.

What happens during egg retrieval?

When the follicles have developed enough to be harvested, the patient attends an appointment  where she is anesthetized and prepared for the procedure. Next, the doctor uses an ultrasound probe to guide a needle through the vaginal wall and into the follicle of the ovary. The thin needle draws the follicle fluid, which is then examined by an embryologist to find the eggs. The whole process takes about 20 minutes.

What happens to the eggs?

In the next step, the harvested eggs are then fertilized. If the sperm from the potential father, or in some cases, anonymous donor, has normal functionality, the eggs and sperm are placed together in a dish with a nutrient fluid, then incubated overnight to fertilize normally. If the sperm functionality is suboptimal, an embryologist uses Intracytoplasmic Sperm Injection to inject a single sperm into a single egg with an extremely precise glass needle.  Once fertilization is complete, the embryos are assessed and prepared to be transferred to the patient’s uterus.

How are the embryos transferred back to the uterus?

The doctor and the patient will discuss the number of embryos to be transferred. The number of successfully fertilized eggs usually determines the number of eggs to be placed in the uterus. Embryos are transferred to the uterus with transabdominal ultrasound guidance. This process does not require anesthesia, but it can cause minor cervical or uterine discomfort. Following transfer, the patient is advised to take at least one days bed rest and two or three additional days of rest, then 10 to 12 days later, two pregnancy tests are scheduled to confirm success. Once two positive tests are completed, an obstetrical ultrasound is ordered to show the sac, fetal pole, yolk sac and fetal heart rate.

Embryoscope©

Built into this technology there is a microscope with a powerful camera that allows the uninterrupted monitoring of the embryo during its first hours of life. In this way, we can keep a close eye on the embryo, from the moment when the oocyte is inseminated and begins to divide into smaller and smaller cells, until it can be transferred to the uterus.
