Lipid Disorders

Steps Involved in IVF:

Procedure Description

For individuals whose lipid levels cannot be managed solely through lifestyle adjustments, a range of medical procedures is available. Standard treatments include medication, therapeutic apheresis, and specialized dietary and lifestyle programs. Here, we focus on these widely used procedures:

  • Medication Therapy
Statins, fibrates, niacin, and PCSK9 inhibitors are among the most commonly prescribed drugs for lipid disorders. Statins, in particular, work by inhibiting an enzyme responsible for cholesterol production in the liver, effectively reducing LDL cholesterol levels. In cases where statins alone are insufficient or not tolerated, fibrates may be prescribed to target triglycerides, and PCSK9 inhibitors are available for patients requiring further LDL reduction.
  • Therapeutic Apheresis
For severe hyperlipidemia cases, particularly familial hypercholesterolemia, therapeutic apheresis may be recommended. This procedure is akin to dialysis for lipids, where blood is filtered to remove excess LDL cholesterol. This is a highly specialized procedure requiring regular sessions to keep lipid levels in check. Therapeutic apheresis is particularly beneficial for patients resistant to conventional medications.
  • Lifestyle Intervention Programs
In combination with medications, therapeutic lifestyle programs involving nutrition counseling, exercise regimens, and behavioral therapy are critical for successful long-term lipid management. Some medical centers offer immersive programs where patients undergo supervised lifestyle adjustments, with a team of dietitians, trainers, and counselors assisting in the adoption of heart-healthy habits.

These procedures aim to reduce LDL and triglyceride levels, prevent plaque buildup in arteries, and ultimately lower the risk of cardiovascular disease. The right approach often involves a combination of these therapies, customized to the patient's lipid profile, overall health, and genetic factors.

Procedure Duration

Each treatment for lipid disorders has its unique duration and frequency:

  • Medication Therapy Duration
Lipid-lowering medications are often a long-term commitment. Patients typically begin with one or two medications, gradually adjusted to achieve target lipid levels. Regular monitoring is essential, with lab tests conducted every 3-6 months to evaluate effectiveness and detect any side effects.
  • Therapeutic Apheresis Schedule
Therapeutic apheresis for lipid disorders, particularly LDL apheresis, generally requires ongoing sessions every 1-2 weeks. Each session can last 2-4 hours, depending on the type of apheresis system used and the patient’s lipid levels. This is a lifelong treatment for certain genetic conditions, making adherence to the schedule crucial for success.
  • Lifestyle Program Duration
Lifestyle programs vary in duration, ranging from 12-week programs to multi-month or year-long interventions. During this period, patients undergo diet planning, physical training, and other behavioral adjustments. Regular follow-up is encouraged even after the intensive phase, with remote consultations or periodic visits to the clinic.

Recovery from these procedures is generally minimal, with medication adjustments and lifestyle interventions fitting into patients' daily routines. Therapeutic apheresis, while more time-intensive, is well-tolerated, allowing patients to resume normal activities post-session.

Risks & Considerations

While managing lipid disorders through medical procedures is effective, it’s essential to be aware of potential risks and considerations:

  • Medication Side Effects: Some lipid-lowering drugs can cause muscle pain, liver enzyme abnormalities, or digestive issues.
  • Frequent Monitoring Requirements: Regular blood tests are needed to assess medication efficacy and detect side effects, requiring ongoing patient commitment.
  • Procedure Accessibility: Therapeutic apheresis is not available in all hospitals, which may limit access depending on geographic location.
  • Long-Term Adherence: Both medications and lifestyle changes require consistent adherence for maximum effectiveness.
  • High Costs for Advanced Therapies: Procedures such as apheresis can be costly, especially for patients without insurance or those in countries where medical costs are high.

How to Choose the Right Doctor and Hospital

Selecting the right healthcare provider for lipid disorder management is crucial. Start by seeking a medical center with a strong focus on cardiovascular health or lipidology. Research the facility’s reputation for lipid management, therapeutic apheresis availability, and quality of care, ensuring it has up-to-date technology and experienced staff.

In terms of selecting a doctor, look for a lipidologist, cardiologist, or endocrinologist with expertise in managing lipid disorders. Confirm the doctor's credentials, years of experience, and treatment approach, preferably finding one who emphasizes a holistic approach that includes lifestyle interventions alongside medical treatments.

Frequently Asked Questions

What actually happens during hyperstimulation of the ovaries?

The patient will take injectable FSH (follicle stimulating hormone) for eight to eleven days, depending on how long the follicles take to mature. This hormone is produced naturally in a woman’s body causing one egg to develop per cycle. Taking the injectable FSH causes several follicles to develop at once, at approximately the same rate. The development is monitored with vaginal ultrasounds and following the patient’s levels of estradiol and progesterone. FSH brand names include Repronex, Follistim, Menopur, Gonal-F and Bravelle. The patient injects herself daily.

What happens during egg retrieval?

When the follicles have developed enough to be harvested, the patient attends an appointment  where she is anesthetized and prepared for the procedure. Next, the doctor uses an ultrasound probe to guide a needle through the vaginal wall and into the follicle of the ovary. The thin needle draws the follicle fluid, which is then examined by an embryologist to find the eggs. The whole process takes about 20 minutes.

What happens to the eggs?

In the next step, the harvested eggs are then fertilized. If the sperm from the potential father, or in some cases, anonymous donor, has normal functionality, the eggs and sperm are placed together in a dish with a nutrient fluid, then incubated overnight to fertilize normally. If the sperm functionality is suboptimal, an embryologist uses Intracytoplasmic Sperm Injection to inject a single sperm into a single egg with an extremely precise glass needle.  Once fertilization is complete, the embryos are assessed and prepared to be transferred to the patient’s uterus.

How are the embryos transferred back to the uterus?

The doctor and the patient will discuss the number of embryos to be transferred. The number of successfully fertilized eggs usually determines the number of eggs to be placed in the uterus. Embryos are transferred to the uterus with transabdominal ultrasound guidance. This process does not require anesthesia, but it can cause minor cervical or uterine discomfort. Following transfer, the patient is advised to take at least one days bed rest and two or three additional days of rest, then 10 to 12 days later, two pregnancy tests are scheduled to confirm success. Once two positive tests are completed, an obstetrical ultrasound is ordered to show the sac, fetal pole, yolk sac and fetal heart rate.

Embryoscope©

Built into this technology there is a microscope with a powerful camera that allows the uninterrupted monitoring of the embryo during its first hours of life. In this way, we can keep a close eye on the embryo, from the moment when the oocyte is inseminated and begins to divide into smaller and smaller cells, until it can be transferred to the uterus.

Patients now have a minimally invasive option. Stem cell therapy for back pain and disc herniations can potentially repair the damaged disc or facet joint, restore function, rehydrate the disc, and ultimately alleviate chronic pain.

Anti-Aging Stem Cell

Hair Loss

Stem cell therapy and PRP therapy have been shown to be most effective for: Those in the early stages of hair loss, patients who are not viable candidates for surgery and women who prefer to avoid hair surgery.

Facial Anti-Aging

Aesthetic Anti-Aging. The Aesthetic Stem Cell Localized Treatment is a non-surgical minimally invasive procedure to enhance the appearance of aging skin and hair restoration. This all-natural technique combines dermal injections of bone marrow or adipose tissue derived stem cells and growth factors.
