Inguinal Hernioplasty

Steps Involved in IVF:

Procedure Description

Inguinal hernioplasty, commonly referred to as hernia repair surgery, is a medical procedure aimed at treating inguinal hernias, which occur when tissue—often part of the intestine—protrudes through a weak spot in the lower abdominal muscles near the groin. This condition can cause discomfort, pain, and in severe cases, complications like strangulation of the intestine, which requires immediate surgical intervention. Hernioplasty is a highly effective method for addressing this issue, involving the reinforcement of the weakened area to prevent future occurrences.

The two primary approaches for inguinal hernioplasty are open surgery and laparoscopic (minimally invasive) surgery. In open hernioplasty, a small incision is made in the groin to access the hernia, allowing the surgeon to push the protruding tissue back into place and secure the area with a synthetic mesh. This mesh acts as a reinforcement, providing added support to the abdominal wall and reducing the likelihood of recurrence. Laparoscopic surgery, on the other hand, involves several small incisions through which the surgeon inserts a laparoscope—a small camera—and specialized instruments. This technique is preferred for its reduced recovery time and minimal scarring, though the choice of procedure depends on the patient’s specific case and the surgeon’s recommendation.

Inguinal hernioplasty is a common and generally safe procedure, and advances in surgical techniques have significantly improved its effectiveness. The addition of mesh in modern surgeries has proven to decrease recurrence rates drastically, making hernioplasty one of the most reliable treatments for inguinal hernias. However, like any surgical procedure, it requires an experienced surgeon, proper aftercare, and attention to individual patient needs.

Procedure Duration

The duration of inguinal hernioplasty can vary based on the surgical approach, the complexity of the hernia, and patient-specific factors. Generally, the procedure itself takes about 30 minutes to two hours. Open hernioplasty is often quicker, typically lasting under an hour, while laparoscopic hernioplasty may take slightly longer due to the additional setup and equipment involved. Anesthesia is administered either as local anesthesia with sedation or general anesthesia, depending on the patient’s health and surgeon’s preference.

After the procedure, patients are moved to a recovery area for observation. The immediate postoperative period is generally short, lasting a few hours, after which patients can often return home the same day. However, in cases of complicated or recurrent hernias, an overnight stay may be necessary to monitor recovery closely. Surgeons typically recommend rest and limited movement in the initial days following the surgery, allowing the body to adapt and heal.

Full recovery from inguinal hernioplasty varies among individuals but usually spans from a few days to a few weeks. Patients are generally advised to avoid heavy lifting and strenuous activities for four to six weeks post-surgery to ensure the surgical area heals properly. Most people can return to light daily activities within a week, but a gradual return to a fully active lifestyle is recommended to reduce the risk of complications or recurrence.

Benefits

  • Effective Hernia Repair: Inguinal hernioplasty has a high success rate in correcting hernias and relieving symptoms.
  • Low Recurrence Rates: The use of mesh reinforcement significantly reduces the likelihood of hernia recurrence.
  • Minimally Invasive Options: Laparoscopic surgery offers a minimally invasive alternative, resulting in faster recovery and minimal scarring.
  • Short Procedure and Recovery Time: The surgery and postoperative recovery are relatively quick, allowing most patients to resume daily activities shortly after.
  • Pain Relief and Improved Quality of Life: The procedure alleviates discomfort, enhances mobility, and reduces the risk of severe complications.

Risks & Considerations

  • Infection: As with any surgical procedure, there is a risk of infection, which requires prompt treatment and care to prevent further complications.
  • Hematoma or Seroma: Patients may develop fluid or blood buildup at the surgical site, which can lead to swelling and discomfort.
  • Chronic Pain: Some individuals may experience lingering pain in the groin area post-surgery, though this is relatively uncommon.
  • Recurrence: Although mesh reinforcement reduces recurrence rates, it is still a possibility, especially in high-risk patients or complex cases.
  • Mesh Complications: In rare instances, the mesh used in hernia repair can lead to complications like adhesion, rejection, or chronic inflammation, which may necessitate further medical intervention.

How to Choose the Right Doctor and Hospital

Selecting the right doctor and hospital for inguinal hernioplasty is crucial for a successful outcome and smooth recovery. Patients should research surgeons who specialize in hernia repairs, ideally those with extensive experience in both open and laparoscopic techniques. Seeking out surgeons with certifications from reputable medical boards or associations adds assurance of their expertise and skill.

When choosing a hospital, look for internationally accredited facilities with a strong track record in patient care and safety. Checking patient reviews and understanding the hospital’s protocols for postoperative care can provide additional confidence in making an informed decision. Opting for a hospital that prioritizes patient support and has a well-established medical tourism program can also make the experience more comfortable and seamless.

Frequently Asked Questions

What actually happens during hyperstimulation of the ovaries?

The patient will take injectable FSH (follicle stimulating hormone) for eight to eleven days, depending on how long the follicles take to mature. This hormone is produced naturally in a woman’s body causing one egg to develop per cycle. Taking the injectable FSH causes several follicles to develop at once, at approximately the same rate. The development is monitored with vaginal ultrasounds and following the patient’s levels of estradiol and progesterone. FSH brand names include Repronex, Follistim, Menopur, Gonal-F and Bravelle. The patient injects herself daily.

What happens during egg retrieval?

When the follicles have developed enough to be harvested, the patient attends an appointment  where she is anesthetized and prepared for the procedure. Next, the doctor uses an ultrasound probe to guide a needle through the vaginal wall and into the follicle of the ovary. The thin needle draws the follicle fluid, which is then examined by an embryologist to find the eggs. The whole process takes about 20 minutes.

What happens to the eggs?

In the next step, the harvested eggs are then fertilized. If the sperm from the potential father, or in some cases, anonymous donor, has normal functionality, the eggs and sperm are placed together in a dish with a nutrient fluid, then incubated overnight to fertilize normally. If the sperm functionality is suboptimal, an embryologist uses Intracytoplasmic Sperm Injection to inject a single sperm into a single egg with an extremely precise glass needle.  Once fertilization is complete, the embryos are assessed and prepared to be transferred to the patient’s uterus.

How are the embryos transferred back to the uterus?

The doctor and the patient will discuss the number of embryos to be transferred. The number of successfully fertilized eggs usually determines the number of eggs to be placed in the uterus. Embryos are transferred to the uterus with transabdominal ultrasound guidance. This process does not require anesthesia, but it can cause minor cervical or uterine discomfort. Following transfer, the patient is advised to take at least one days bed rest and two or three additional days of rest, then 10 to 12 days later, two pregnancy tests are scheduled to confirm success. Once two positive tests are completed, an obstetrical ultrasound is ordered to show the sac, fetal pole, yolk sac and fetal heart rate.

Embryoscope©

Built into this technology there is a microscope with a powerful camera that allows the uninterrupted monitoring of the embryo during its first hours of life. In this way, we can keep a close eye on the embryo, from the moment when the oocyte is inseminated and begins to divide into smaller and smaller cells, until it can be transferred to the uterus.
