Crowns and Bridges – Porcelain Fused to Metal (Per Tooth)

Steps Involved in IVF:

Procedure Description

Porcelain fused to metal crowns and bridges are designed to restore both the functionality and aesthetics of damaged or missing teeth. They are commonly used to cover or replace teeth that have been compromised by decay, fractures, or extensive dental work.

The procedure begins with a thorough examination by a dentist or prosthodontist, who will assess the condition of the tooth (or teeth) requiring restoration. If a tooth is severely damaged but still has a viable root, a crown is used to cover it. However, if the tooth is missing, a dental bridge, which consists of two crowns anchoring an artificial tooth (pontic) in the middle, may be recommended.

Porcelain fused to metal crowns and bridges are created by fusing a layer of porcelain to a metal base. The metal provides strength and durability, while the porcelain gives the crown or bridge a natural, tooth-like appearance. The metal core, usually made from alloys like nickel or cobalt-chromium, ensures that the restoration can withstand chewing forces, while the porcelain exterior blends seamlessly with the surrounding teeth.

This procedure is commonly chosen by patients seeking long-lasting solutions for damaged or missing teeth. It is ideal for both anterior (front) and posterior (back) teeth, as it combines durability with an attractive appearance.

Procedure Duration

The process of receiving a porcelain fused to metal crown or bridge usually takes two to three visits to a dental clinic, spanning a few weeks. Here’s a breakdown of the phases involved:

1. Initial Consultation and Tooth Preparation:

During the first visit, the dentist will conduct a thorough examination of your oral health, including taking X-rays to determine if a root canal is needed before placing the crown. The dentist will then prepare the tooth by removing any decay and shaping it to fit the crown. If a dental bridge is needed, the adjacent teeth will be prepared to support the bridge. This preparation often involves reshaping the teeth by filing down their outer surface.

2. Impressions and Temporary Crowns:

Once the teeth have been prepared, the dentist will take impressions of your teeth to create a custom-fit crown or bridge. These impressions are sent to a dental laboratory where the final restoration will be created. In the meantime, a temporary crown or bridge will be placed to protect the tooth until the final restoration is ready. This temporary phase typically lasts about one to two weeks.

3. Final Fitting and Placement:

During your second visit, the dentist will remove the temporary restoration and check the fit of the permanent porcelain fused to metal crown or bridge. Once the fit is confirmed, the restoration is cemented in place. You may be advised to return for a follow-up appointment to ensure the crown or bridge is functioning correctly and does not require any adjustments.

Recovery time from this procedure is minimal, though patients may experience mild discomfort or sensitivity in the treated area for a few days after placement. Most patients can return to their normal activities immediately after the final fitting.

Benefits

Porcelain fused to metal crowns and bridges offer several advantages, making them a popular choice among patients. Here are some key benefits:

  • Durability and Strength: The metal base provides a strong foundation, allowing the crown or bridge to withstand the forces of chewing and biting.
  • Natural Appearance: The porcelain exterior mimics the look of natural teeth, making PFM crowns and bridges a visually appealing option
  • Long-lasting: With proper care, these restorations can last 10 to 15 years or longer.
  • Versatility: They can be used for both anterior and posterior teeth, offering solutions for various dental problems.
  • Cost-effective: Compared to all-porcelain crowns, porcelain fused to metal crowns are often more affordable, while still offering a high level of aesthetics and functionality.

Risks & Considerations

While porcelain fused to metal crowns and bridges are generally safe and effective, there are some risks and considerations to be aware of:

  • Allergic Reactions: Some patients may be allergic to the metal used in the crown or bridge. It’s important to discuss any known allergies with your dentist beforehand.
  • Gum Recession: Over time, the gums around the crown or bridge may recede, potentially exposing the metal layer and affecting the aesthetic result.
  • Tooth Sensitivity: Patients may experience temporary sensitivity to hot or cold foods after the procedure, though this usually resolves within a few weeks.
  • Risk of Fracture: Although porcelain is durable, it is not as strong as natural tooth enamel, and the porcelain layer may crack or chip under extreme force.
  • Cement Failure: In rare cases, the cement holding the crown or bridge in place may weaken, causing the restoration to become loose.

How to Choose the Right Doctor and Hospital

Choosing the right doctor and hospital for your dental procedure is crucial to ensuring a successful outcome. Here are some tips to guide you in making an informed decision:

  • Credentials and Experience: Ensure the dentist performing the procedure has the necessary qualifications and certifications. Look for professionals who specialize in restorative dentistry, prosthodontics, or have extensive experience in performing porcelain fused to metal restorations.
  • Facility Accreditation: Opt for clinics that are internationally accredited, as this ensures they meet high standards of care and safety. Even though we avoid specific accreditations in this article, it's important to ensure the clinic complies with recognized international health and safety standards.

Frequently Asked Questions

What actually happens during hyperstimulation of the ovaries?

The patient will take injectable FSH (follicle stimulating hormone) for eight to eleven days, depending on how long the follicles take to mature. This hormone is produced naturally in a woman’s body causing one egg to develop per cycle. Taking the injectable FSH causes several follicles to develop at once, at approximately the same rate. The development is monitored with vaginal ultrasounds and following the patient’s levels of estradiol and progesterone. FSH brand names include Repronex, Follistim, Menopur, Gonal-F and Bravelle. The patient injects herself daily.

What happens during egg retrieval?

When the follicles have developed enough to be harvested, the patient attends an appointment  where she is anesthetized and prepared for the procedure. Next, the doctor uses an ultrasound probe to guide a needle through the vaginal wall and into the follicle of the ovary. The thin needle draws the follicle fluid, which is then examined by an embryologist to find the eggs. The whole process takes about 20 minutes.

What happens to the eggs?

In the next step, the harvested eggs are then fertilized. If the sperm from the potential father, or in some cases, anonymous donor, has normal functionality, the eggs and sperm are placed together in a dish with a nutrient fluid, then incubated overnight to fertilize normally. If the sperm functionality is suboptimal, an embryologist uses Intracytoplasmic Sperm Injection to inject a single sperm into a single egg with an extremely precise glass needle.  Once fertilization is complete, the embryos are assessed and prepared to be transferred to the patient’s uterus.

How are the embryos transferred back to the uterus?

The doctor and the patient will discuss the number of embryos to be transferred. The number of successfully fertilized eggs usually determines the number of eggs to be placed in the uterus. Embryos are transferred to the uterus with transabdominal ultrasound guidance. This process does not require anesthesia, but it can cause minor cervical or uterine discomfort. Following transfer, the patient is advised to take at least one days bed rest and two or three additional days of rest, then 10 to 12 days later, two pregnancy tests are scheduled to confirm success. Once two positive tests are completed, an obstetrical ultrasound is ordered to show the sac, fetal pole, yolk sac and fetal heart rate.

Embryoscope©

Built into this technology there is a microscope with a powerful camera that allows the uninterrupted monitoring of the embryo during its first hours of life. In this way, we can keep a close eye on the embryo, from the moment when the oocyte is inseminated and begins to divide into smaller and smaller cells, until it can be transferred to the uterus.
