Buccal Fat Removal

Steps Involved in IVF:

Procedure Description

Buccal fat removal, also known as buccal lipectomy or cheek reduction surgery, is a cosmetic procedure designed to thin the cheeks and enhance the contours of the face. This minimally invasive surgery targets the buccal fat pads located between the facial muscles in the hollow area beneath the cheekbones. The procedure is ideal for individuals seeking a more sculpted and defined facial appearance, often enhancing the natural structure of the cheekbones and jawline.

During buccal fat removal, a plastic or cosmetic surgeon makes a small incision inside the mouth, near the upper molars, to access the buccal fat pads. After making the incision, the surgeon gently extracts the desired amount of fat to achieve the intended contour. This approach results in a more streamlined appearance, creating a slimmer and more defined facial structure. Since the incisions are made inside the mouth, there are no visible scars, making it an attractive option for those looking to improve their facial aesthetics discreetly.

Patients who opt for buccal fat removal often have a "fuller" or "rounder" face, which can be due to genetics, age, or weight. It is important to note that while the procedure helps contour the face, it does not significantly alter the overall facial structure but rather emphasizes the natural angles and features. Candidates for the surgery are generally those in good health, non-smokers, and with a stable weight.

Procedure Duration

The buccal fat removal procedure is relatively quick, typically taking about 30 to 60 minutes. Since it is performed on an outpatient basis, patients can return home on the same day. Local anesthesia or sedation is used to numb the area, allowing the patient to remain comfortable during the procedure.

Once the anesthesia is administered, the surgeon makes a small incision inside the mouth and carefully removes the appropriate amount of buccal fat from each cheek. The process is meticulous to ensure that both sides are symmetrical and aesthetically balanced. After the fat pads are removed, the incisions are closed with absorbable sutures, eliminating the need for a follow-up to remove stitches.

The recovery period following buccal fat removal is typically short. Most patients experience some swelling, bruising, or discomfort in the days following the surgery, but these symptoms usually subside within one to two weeks. Patients are advised to stick to a soft diet initially and maintain good oral hygiene to avoid infections. Full results from the procedure are generally visible within three months as the swelling diminishes and the cheeks settle into their new contour.

Benefits

  • Enhanced Facial Contour: The removal of excess buccal fat can accentuate the cheekbones and jawline, giving a slimmer and more sculpted facial appearance.
  • Quick and Minimally Invasive: The procedure typically lasts an hour or less and is performed on an outpatient basis, reducing overall downtime.
  • No Visible Scarring: Since the incisions are made inside the mouth, there are no external scars, allowing for a discreet enhancement.
  • Permanent Results: Once the buccal fat pads are removed, they do not return, providing a long-lasting enhancement to the facial structure.
  • Boost in Confidence: Many patients experience an increase in self-esteem and satisfaction with their facial appearance post-procedure.

Risks & Considerations

  • Infection: As with any surgery, there is a risk of infection. Proper post-operative care, including maintaining oral hygiene, is crucial to minimize this risk.
  • Swelling and Bruising: Mild swelling and bruising are common post-surgery and may take up to two weeks to subside completely.
  • Asymmetry: There is a small chance of asymmetry if the buccal fat is not evenly removed from both sides of the face, which may require a revision surgery.
  • Facial Nerve Injury: Although rare, there is a risk of damage to the facial nerve during the procedure, which could potentially affect facial movements.
  • Undesired Outcome: Patients may not achieve the desired look or may feel that the results are not as significant as expected. It is essential to have realistic expectations and discuss the potential outcomes thoroughly with the surgeon beforehand.

How to Choose the Right Doctor and Hospital

When considering buccal fat removal, selecting a qualified and experienced plastic or cosmetic surgeon is crucial. Look for a board-certified surgeon who specializes in facial contouring and has an extensive portfolio of successful buccal fat removal cases. A consultation should include an in-depth discussion of your goals, a review of before-and-after photos, and an assessment of your facial structure to determine the best approach.

In addition to the surgeon's qualifications, it is essential to choose a hospital or clinic with high standards of safety, hygiene, and patient care. Accredited facilities with modern equipment and comprehensive pre- and post-operative care are crucial for a smooth procedure and recovery. Always verify the facility's reputation through patient reviews and ensure it complies with international healthcare standards.

Frequently Asked Questions

What actually happens during hyperstimulation of the ovaries?

The patient will take injectable FSH (follicle stimulating hormone) for eight to eleven days, depending on how long the follicles take to mature. This hormone is produced naturally in a woman’s body causing one egg to develop per cycle. Taking the injectable FSH causes several follicles to develop at once, at approximately the same rate. The development is monitored with vaginal ultrasounds and following the patient’s levels of estradiol and progesterone. FSH brand names include Repronex, Follistim, Menopur, Gonal-F and Bravelle. The patient injects herself daily.

What happens during egg retrieval?

When the follicles have developed enough to be harvested, the patient attends an appointment  where she is anesthetized and prepared for the procedure. Next, the doctor uses an ultrasound probe to guide a needle through the vaginal wall and into the follicle of the ovary. The thin needle draws the follicle fluid, which is then examined by an embryologist to find the eggs. The whole process takes about 20 minutes.

What happens to the eggs?

In the next step, the harvested eggs are then fertilized. If the sperm from the potential father, or in some cases, anonymous donor, has normal functionality, the eggs and sperm are placed together in a dish with a nutrient fluid, then incubated overnight to fertilize normally. If the sperm functionality is suboptimal, an embryologist uses Intracytoplasmic Sperm Injection to inject a single sperm into a single egg with an extremely precise glass needle.  Once fertilization is complete, the embryos are assessed and prepared to be transferred to the patient’s uterus.

How are the embryos transferred back to the uterus?

The doctor and the patient will discuss the number of embryos to be transferred. The number of successfully fertilized eggs usually determines the number of eggs to be placed in the uterus. Embryos are transferred to the uterus with transabdominal ultrasound guidance. This process does not require anesthesia, but it can cause minor cervical or uterine discomfort. Following transfer, the patient is advised to take at least one days bed rest and two or three additional days of rest, then 10 to 12 days later, two pregnancy tests are scheduled to confirm success. Once two positive tests are completed, an obstetrical ultrasound is ordered to show the sac, fetal pole, yolk sac and fetal heart rate.

Embryoscope©

Built into this technology there is a microscope with a powerful camera that allows the uninterrupted monitoring of the embryo during its first hours of life. In this way, we can keep a close eye on the embryo, from the moment when the oocyte is inseminated and begins to divide into smaller and smaller cells, until it can be transferred to the uterus.
