Bone Tumor Resection

Steps Involved in IVF:

Procedure Description

Bone tumor resection is a surgical procedure designed to remove tumors from the bone, whether they are benign (non-cancerous) or malignant (cancerous). Bone tumors can develop in any part of the skeletal system and may arise from the bone itself or spread to the bone from other organs (a process known as metastasis). Resection is often necessary when the tumor threatens the stability and function of the bone or poses a risk of spreading to other areas of the body.

The procedure can be complex due to the proximity of vital structures such as blood vessels, nerves, and muscles. It typically involves the complete or partial removal of the affected bone tissue, followed by reconstruction to maintain the integrity of the skeletal system. In cases where the tumor is cancerous, the goal is to remove the entire tumor and a margin of healthy tissue to prevent recurrence.

Depending on the size, type, and location of the tumor, various surgical techniques are employed. Some surgeries involve minimally invasive approaches, while others require open surgery. Surgeons may use advanced imaging technologies, such as MRI and CT scans, to guide the resection process, ensuring precision in removing the tumor while minimizing damage to surrounding tissues.

In more extensive cases, reconstructive surgery may follow to restore the bone’s function and appearance. This may involve the use of metal plates, screws, bone grafts, or even prosthetic devices. The complexity of the surgery depends on several factors, including the size and location of the tumor and the patient’s overall health.

Procedure Duration

Bone tumor resection is a multi-phase process that typically involves pre-operative preparation, the surgical procedure itself, and post-operative recovery.

  • Pre-operative Phase:
This phase involves detailed diagnostic evaluations such as imaging tests (X-rays, MRI, CT scans) and biopsy to determine the type, size, and exact location of the tumor. Patients undergo medical evaluations to ensure they are fit for surgery. Depending on the complexity of the tumor, surgeons may collaborate with oncologists, radiologists, and orthopedic specialists to create a comprehensive treatment plan.
  • Surgical Phase:
The duration of the surgery depends on the size and location of the tumor and the complexity of the resection. A typical bone tumor resection procedure may last anywhere from 2 to 6 hours. In cases where the bone is extensively affected, surgeons may perform additional reconstructive procedures, which can extend the time in surgery. Minimally invasive approaches, where applicable, can reduce the length of the surgery.
  • Post-operative Recovery:
The recovery phase varies based on the extent of the surgery and the patient's overall health. For minor resections, recovery may take 4 to 6 weeks, while more extensive surgeries involving reconstruction may require several months of rehabilitation. Physical therapy is often recommended to help patients regain strength and mobility. Close monitoring through follow-up appointments ensures proper healing and addresses any potential complications early on.

Benefits

  • Tumor Removal: Effective elimination of benign or malignant tumors, reducing the risk of metastasis or recurrence.
  • Preservation of Bone Function: Advanced surgical techniques allow for the preservation or restoration of bone stability and function.
  • Pain Relief: Tumor resection can significantly alleviate pain caused by the tumor pressing on nerves or weakening the bone.
  • Improved Quality of Life: Removing the tumor can improve mobility and overall quality of life, especially in cases where the tumor causes significant discomfort.
  • Reconstruction Possibilities: Surgeons can reconstruct affected bones using grafts or prosthetics, ensuring better post-surgical outcomes.

Risks & Considerations

  • Infection: Post-surgical infections are a risk, especially in complex surgeries involving bone reconstruction. Proper hygiene and wound care can minimize this risk.
  • Damage to Surrounding Tissue: During the resection, there is a potential risk of damage to surrounding nerves, blood vessels, or muscles, which can affect mobility and function.
  • Recurrence: In cases of malignant tumors, there is a risk that not all cancerous cells are removed, leading to recurrence. Follow-up care is essential to monitor and address any signs of tumor regrowth.
  • Prolonged Recovery Time: Depending on the complexity of the surgery, recovery time can be extended, requiring months of rehabilitation and physical therapy.
  • Post-surgical Complications: Blood clots, excessive bleeding, or poor wound healing are potential risks associated with bone tumor resection, particularly in patients with other underlying health conditions.

How to Choose the Right Doctor and Hospital

When selecting a doctor or hospital for bone tumor resection, it’s essential to consider several factors to ensure you receive the best care possible. First, look for a surgeon with extensive experience in orthopedic oncology, as this specialized field requires in-depth knowledge of both cancer and bone surgery. Verify the surgeon’s credentials, including board certifications and their specific expertise in bone tumor resections.

In terms of hospital selection, opt for facilities that have a strong focus on orthopedic surgery and oncology. Ensure that the hospital is equipped with advanced diagnostic and surgical technologies, as this will impact the precision and success of the procedure. Reading patient reviews and understanding the hospital’s success rate with similar procedures can also help guide your decision. Always inquire about the post-operative care and rehabilitation programs offered, as these are crucial for a successful recovery.

Frequently Asked Questions

What actually happens during hyperstimulation of the ovaries?

The patient will take injectable FSH (follicle stimulating hormone) for eight to eleven days, depending on how long the follicles take to mature. This hormone is produced naturally in a woman’s body causing one egg to develop per cycle. Taking the injectable FSH causes several follicles to develop at once, at approximately the same rate. The development is monitored with vaginal ultrasounds and following the patient’s levels of estradiol and progesterone. FSH brand names include Repronex, Follistim, Menopur, Gonal-F and Bravelle. The patient injects herself daily.

What happens during egg retrieval?

When the follicles have developed enough to be harvested, the patient attends an appointment  where she is anesthetized and prepared for the procedure. Next, the doctor uses an ultrasound probe to guide a needle through the vaginal wall and into the follicle of the ovary. The thin needle draws the follicle fluid, which is then examined by an embryologist to find the eggs. The whole process takes about 20 minutes.

What happens to the eggs?

In the next step, the harvested eggs are then fertilized. If the sperm from the potential father, or in some cases, anonymous donor, has normal functionality, the eggs and sperm are placed together in a dish with a nutrient fluid, then incubated overnight to fertilize normally. If the sperm functionality is suboptimal, an embryologist uses Intracytoplasmic Sperm Injection to inject a single sperm into a single egg with an extremely precise glass needle.  Once fertilization is complete, the embryos are assessed and prepared to be transferred to the patient’s uterus.

How are the embryos transferred back to the uterus?

The doctor and the patient will discuss the number of embryos to be transferred. The number of successfully fertilized eggs usually determines the number of eggs to be placed in the uterus. Embryos are transferred to the uterus with transabdominal ultrasound guidance. This process does not require anesthesia, but it can cause minor cervical or uterine discomfort. Following transfer, the patient is advised to take at least one days bed rest and two or three additional days of rest, then 10 to 12 days later, two pregnancy tests are scheduled to confirm success. Once two positive tests are completed, an obstetrical ultrasound is ordered to show the sac, fetal pole, yolk sac and fetal heart rate.

Embryoscope©

Built into this technology there is a microscope with a powerful camera that allows the uninterrupted monitoring of the embryo during its first hours of life. In this way, we can keep a close eye on the embryo, from the moment when the oocyte is inseminated and begins to divide into smaller and smaller cells, until it can be transferred to the uterus.
