Arthroscopic Shoulder

Steps Involved in IVF:

Procedure Description

Arthroscopic shoulder surgery is a minimally invasive procedure used to diagnose and treat a range of shoulder conditions. It involves the use of an arthroscope, a small camera inserted into the shoulder joint through tiny incisions. This allows the surgeon to view the interior of the joint on a monitor and perform necessary repairs, such as removing damaged tissue or repairing torn ligaments, with precision. The arthroscope provides real-time visualization, enhancing the surgeon’s ability to work in confined spaces without the need for large incisions.

This procedure is commonly performed to address conditions such as rotator cuff tears, shoulder impingement, labral tears, and arthritis. Arthroscopic shoulder surgery is often preferred over traditional open surgery because it results in smaller incisions, less post-operative pain, and a quicker recovery. The surgeon can also perform complex joint reconstructions with minimal disruption to surrounding tissues.

Arthroscopic shoulder surgery is often performed on an outpatient basis, meaning that most patients can go home the same day. However, the complexity of the condition being treated can influence the procedure’s duration and recovery time. This minimally invasive approach has gained popularity due to its effectiveness in restoring shoulder function and reducing recovery time compared to traditional methods.

Procedure Duration

The duration of an arthroscopic shoulder surgery depends largely on the specific condition being treated and the complexity of the procedure. On average, the surgery can take anywhere from one to three hours. For example, simple diagnostic procedures might be completed within an hour, while more involved repairs, such as rotator cuff or labral repairs, may take closer to two or three hours.

Before the procedure, the patient will undergo a thorough evaluation, including imaging studies like MRI or X-ray, to identify the issue and plan the surgery accordingly. During the procedure, the patient is usually placed under general anesthesia or a nerve block, and the surgeon makes small incisions to insert the arthroscope and surgical tools. These incisions are typically less than one centimeter in size.

Recovery time after arthroscopic shoulder surgery can vary. While the surgery is minimally invasive, healing still takes time. Most patients can expect to wear a sling for several days to weeks, depending on the complexity of the surgery. Physical therapy is a crucial part of recovery, often starting within a few days post-surgery. Full recovery can take anywhere from several weeks to a few months, with gradual improvements in shoulder strength and range of motion over time.

Benefits

  • Minimally invasive: Small incisions reduce scarring and post-operative pain.
  • Faster recovery: Shorter recovery time compared to open surgery.
  • Outpatient procedure: Many patients can go home the same day of surgery.
  • Improved joint mobility: Effective in restoring range of motion and shoulder function.
  • Less tissue damage: Precision tools cause minimal disruption to surrounding tissues.

Risks & Considerations

  • Infection: Although rare, infections can occur at the surgical site. Surgeons take extensive precautions, but it is a risk with any surgical procedure.
  • Nerve injury: There is a small risk of damage to the nerves surrounding the shoulder, which can lead to temporary or permanent weakness or numbness.
  • Stiffness: Some patients may experience stiffness in the shoulder joint post-surgery, requiring extended physical therapy to regain full range of motion.
  • Failure of the repair: In some cases, the shoulder may not heal as expected, or the repair may not hold, leading to the need for additional surgery.
  • Blood clots: There is a small risk of developing blood clots in the arm or shoulder, which can lead to complications if not promptly treated.

How to Choose the Right Doctor and Hospital

Selecting the right doctor and hospital for arthroscopic shoulder surgery is critical to ensure a successful outcome. Begin by researching surgeons who specialize in shoulder arthroscopy and have extensive experience in treating the specific condition. Look for surgeons with advanced training in orthopedic surgery and certifications that highlight their expertise in minimally invasive procedures. Patient reviews, outcomes, and success rates are also important factors to consider.

When choosing a hospital, focus on facilities that offer advanced medical technology and state-of-the-art operating rooms. A hospital’s reputation for orthopedic care, along with its accreditation by international healthcare organizations, can provide reassurance of the quality of care. Additionally, ensure that the hospital offers comprehensive post-operative care, including rehabilitation services and physical therapy, which are essential components of a successful recovery.

Frequently Asked Questions

What actually happens during hyperstimulation of the ovaries?

The patient will take injectable FSH (follicle stimulating hormone) for eight to eleven days, depending on how long the follicles take to mature. This hormone is produced naturally in a woman’s body causing one egg to develop per cycle. Taking the injectable FSH causes several follicles to develop at once, at approximately the same rate. The development is monitored with vaginal ultrasounds and following the patient’s levels of estradiol and progesterone. FSH brand names include Repronex, Follistim, Menopur, Gonal-F and Bravelle. The patient injects herself daily.

What happens during egg retrieval?

When the follicles have developed enough to be harvested, the patient attends an appointment  where she is anesthetized and prepared for the procedure. Next, the doctor uses an ultrasound probe to guide a needle through the vaginal wall and into the follicle of the ovary. The thin needle draws the follicle fluid, which is then examined by an embryologist to find the eggs. The whole process takes about 20 minutes.

What happens to the eggs?

In the next step, the harvested eggs are then fertilized. If the sperm from the potential father, or in some cases, anonymous donor, has normal functionality, the eggs and sperm are placed together in a dish with a nutrient fluid, then incubated overnight to fertilize normally. If the sperm functionality is suboptimal, an embryologist uses Intracytoplasmic Sperm Injection to inject a single sperm into a single egg with an extremely precise glass needle.  Once fertilization is complete, the embryos are assessed and prepared to be transferred to the patient’s uterus.

How are the embryos transferred back to the uterus?

The doctor and the patient will discuss the number of embryos to be transferred. The number of successfully fertilized eggs usually determines the number of eggs to be placed in the uterus. Embryos are transferred to the uterus with transabdominal ultrasound guidance. This process does not require anesthesia, but it can cause minor cervical or uterine discomfort. Following transfer, the patient is advised to take at least one days bed rest and two or three additional days of rest, then 10 to 12 days later, two pregnancy tests are scheduled to confirm success. Once two positive tests are completed, an obstetrical ultrasound is ordered to show the sac, fetal pole, yolk sac and fetal heart rate.

Embryoscope©

Built into this technology there is a microscope with a powerful camera that allows the uninterrupted monitoring of the embryo during its first hours of life. In this way, we can keep a close eye on the embryo, from the moment when the oocyte is inseminated and begins to divide into smaller and smaller cells, until it can be transferred to the uterus.
